Based on LPA V. Wheatley observation and record review, the licensee did not comply with the section cited above in that Staff #3 was observed supervising children alone without qualifications. This poses/posed a potential health, safety or personal rights risk to persons in care. Staff #1 teaches the children French language. The licensee states that Staff #3 worked with children in another country, however there is no paperwork to show proof of experience or qualification. Licensee states that Staff #3 is enrolled in a child development class currently for winter session.

Based on LPA V. Wheatley observation and record review, the licensee did not comply with the section cited above in that LPA did not observe Staff #3 teaching qualifications, education, etc in the file. This poses/posed a potential health, safety or personal rights risk to persons in care.

(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (2) Request a transfer of a criminal record clearance as specified in Section 101170(f) or

Based on observation, record review and information obtained the facility did not comply with the section cited above,19 children were left under the supervision of an uncleared staff (S2) during today’s visit from 8:45am to12:30pm ., which poses/posed an immediate health, safety or personal rights risk to persons in care.
