Based on observation the Licensee did not store cleaning solutions in an area inaccessible to children in care. The Licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee took cleaning solutions and stored them inaccesible to children in off limits bedroom at time of inspection.
Based on LPa observation and interview Adult 1 (A1) was present at the facility who was not observed on the personnel clearance. Licensee confirms she is not cleared, but visiting the facility. Licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee states daughter is in the vehicle. LPAS observed daughter go to the car and wait in the vehicle parked outside the facility.
Based on observation Licensee did not meet above criteria with the play yard gates with observable thick layer of dirt. A large filled bag of trash in the outdoor play area next to toys. Bathroom floors wet with dark foot print stains. Cockroaches crawling on the sink bowl, and in the top drawer. Kitchen piled with food and cleaning solutions next to each other on the same shelf., the licensee did not comply with the section cited above in the overall facility which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have grandson help clean outdoor yard. The Licensee will provide proof of extermination to LPA with a picture of receipt sent by email or text.
Based on LPA observations, at 9:53 AM LPA observed bathroom that children will use with under cabinet sink that has inoperable child safety lock containing clorox and other cleaning solutions making these items accessible to children in care which is an immediate health, safety, and personal rights risk to children in care.
Resolution: Licensee states she will replace the lock and provide photo proof to LPA by text by the POC date.
Based on LPA record review and interview, Licensee's Assistant did not have immunization records available for review at time of inspection. Licensee asked Assistant about immunization records, which Assistant confirms she has not yet provided at this time. This poses a potential health, safety, and personal rights risk to children in care.
Resolution: Licensee states she will provide proof of immunization records to LPA via text by POC date.
Based on LPA observations, at 9:58 AM LPA observed wooden play kitchen that LPA observed children using in living room to have a sticky feeling and visibly dirty (photos taken). This poses a potential health, safety, and personal rights risk to children in care.
Resolution: Licensee states she will clean up wooden play kitchen and provide photo proof to LPA via text by POC date.
Based on LPA record review and interview, Licensee has expired Mandated Reporter training and Assistants did not have proof of completion available at time of inspection. This is a potential health, safety, and personal rights risk to children in care.
Resolution: Licensee states they will all complete mandated reporter training and provide proof of completion to LPA by POC date via text.
