Based on observation, interview, record review, the licensee did not comply with the section cited above as LPA was not able to review children or staff files during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: License will send LPA proof of files for S1 - S5 and all 14 children who are enrolled at the facility by the POC date

Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 3 staff did not have criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA obsereved S2 leave the home during inspection.

Based on observation, interview, record review, the licensee did not comply with the section cited above as the licensee temporary absences exceed more than 20 percent of the hours that the facility is providing care per day, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall write a declaration stating that she understands and will comply with the terms and conditions of her license which includes that temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. Licensee will send declaration to LPA by POC date.

Based on observation, interview, record review, the licensee did not comply with the section cited above as LPA did not observe any licensing requirements posted for parents to observe, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will post her licensing board with all of the licensing requirements and send a picture to LPA by POC date.

Based on observation, interview, record review, the licensee did not comply with the section cited above in LPA did not observe pediatric cardiopulmonary resuscitation and pediatric first aid certificates for any adult present during the inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall provide proof to LPA of current first aid and CPR for at least one staff member who was present during the inspection.

Based on observation, interview, record review, the licensee did not comply with the section cited above as the licensee did not inform the department that she will not be present for more that 20 percent of the hours that the facility is providing care per day, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall write a declaration stating that she understands and will inform the department when she is not going to be present for more than 20 percent of the hours that the facility is providing care per day. Licensee will send declaration to LPA by POC date.

Based on observations , record review and interview the licensees assistant did not comply with the section cited above. LPAs reviewed the assistant record and did not see proof of immunization required . Licensees assistant missing Pertussis, MMR vaccines , which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email proof of immunization to the LPA by POC date .
