Based on observation, the licensee did not comply with the section cited above in LPA observed the fire extinguisher indicated fully charged; however, it was last serviced 06/15/21, per the attached tag which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee's assistant, they will contact the company to obtain a service tag or service the fire extinguisher (if needed) and will submit proof to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in LPA observed child 1 (C1) and C2 did not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee's assistant, proof of immunizations will be submitted to LPA by POC due date.
