Based upon information received from the interview with Staff #1 and written statements from staff and witnesses, it was determined that Staff #1 left children without the supervision of a teacher during the incidences that occurred on 05/01/2025, 09/15/2025, and 09/19/2025 which poses an immediate risk(s) to the health, safety, or personal rights of children in care.
interviews conducted and disclosures made indicating that Staff #3 was not providing adequate care and visual supervision when Child #1 was left alone standing near the restroom without supervision. This is an potential risk to the health and safety of children in care.
Based on observation and interview, the licensee did not comply with the section cited above in LPA did not observe a carbon monoxide detector in the facility. Per principal, there are no carbon monoxide detectors in the facility. which poses a potential health, safety or personal rights risk to persons in care.

The licensee did not comply with the section cited above in that Staff #1 physically moved Child 1 and Child 2 by grabbing them under their arms and pulling them across the room which poses an immediate health, safety, or personal rights risk to persons in care.
The incident involves a two year old child wetting on herself and not having a change of clothing and staff had to find clothing around center to change child. Child is to have changes of clothes at center this can pose a health & safety hazard.

interviews conducted and disclosures made indicating that Staff #4 violated the personal rights of Child #2 when Child #3 observed Staff #4 pulled on Child#2 hoodie in order to get Child #2 to lay back down on their cot during nap time and Staff #4 violated the personal rights of Child #2 when Staff #5 and Staff #6 observe Staff #4 disclosed that in reaction to Child#2 stating to Staff #4 “I hate you.” Staff #4 responded “I could hate you too.” which poses an immediate health, safety, and personal rights risk to children in care.
