No child(ren) shall be left without the supervision of a teacher at any time, except as specified... Supervision shall include visual observation. This requirement was not met as evidence by: Based on interviews conducted with S1-S3, S1 and S2 did not observe the incident occurr and S1 was not aware of the incident until C1 disclosed what occurred. This poses an immediate health and safety risk to children in care.

Based on interviews conducted with C2 and C4, C2 disclosed that S4 grabbed them by the arm and pulled them. C4 demonstrated to LPA how S6 kicked and stated that it hurt when the incident occurred, this poses/posed a potential health and safety risk to children in care.
