Based on observation, interview and record review, the licensee did not comply with the section cited above in having an uncleared assistant in the home which poses an immediate health, safety or personal rights risk to persons in care. LPA was unable to find the uncleared assistant on the Guardian Employee Roster or the Person Search.
Resolution: Licensee understands that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance. Licensee will provide proof of criminal record clearance for assistant Zhimin Zhang to LPA via email by due date. Additionally, agrees to not have Ms. Zhang or anyone else in the home until fingerprints have been cleared and associated to the home.
Based on record review, the licensee did not comply with the section cited above in being unable to provide current Mandated Reporter Training certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the Mandated Reporter Training and will email certification (Proof of Completion) to LPA by POC due date. https://www.mandatedreporterca.com/
