LPAs observed both toilets in the children's restroom were operable on today's date. Per Staff, one toilet was clogged one day last week, date unknown, and toilet was fixed the same day. License capacity is for 15 children and according to staff one toilet was still available for children. While LPAs toured the facility there was no foul odor in the classroom. During the investigation LPAs obtained a copy of the children's roster, Personnel Report, Sign in/out sheets, took pictures and conducted interviews with four staff. LPAs observed both toilets in the children's restroom were operable on today's date. Per Staff, one toilet was clogged one day last week, date unknown, and toilet was fixed the same day. License capacity is for 15 children and according to staff one toilet was still available for children. While LPAs toured the facility there was no foul odor in the classroom. There were eight children and two staff present when the visit began. During the investigation LPAs obtained a copy of the children's roster, Personnel Report, Sign in/out sheets, took pictures and conducted interviews with four staff. LPAs observed both toilets in the children's restroom were operable on today's date. Per Staff, one toilet was clogged one day last week, date unknown, and toilet was fixed the same day. License capacity is for 15 children and according to staff one toilet was still available for children. While LPAs toured the facility there was no foul odor in the classroom.
