Based on observation, interview, record review, the licensee did not comply with the section cited above as the facility did not obtain a rescue pole body hook, functioning pool alarm and swimming pool daily inspection log, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will sent LPA proof functioning pool alarm, attched rescue body hook to pole and daily pool inspection log to LPA by POC date of 10/03/2025

Based on observation, interview, record review, the licensee did not comply with the section cited above in 2 out of 4 staff did not obtain proof of completed mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure that S2 and S4 have completed mandated reporter training. Licensee will send proof to LPA by POC date of 09/29/2025.

Based on observation, interview, record review, the licensee did not comply with the section cited above as in 2 out of 4 staff did not have proof of measales, pertissis and TB immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA proof that S2 has been immunized against of measales and pertissis and cleared for TB. Licensee will also send proof that S3 has been cleared for TB.

Based on observation, interview, record review, the licensee did not comply with the section cited above in C5 was placed in a crib during nap time so C5 does not disturb other napping children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will write a declaration stating that they understand that will no longer place a child in a crib when they are able to climb out and to prevent disturbances to other children.
