This is a preschool license Head Start program serving children ages 3 until entry of into kindergarten. License capacity serving 40 children. The facility operates Monday – Friday from 8:00 AM – 2:00 PM. Per Mina Rodriguez, Lead Teacher, there are 16 children currently enrolled. Sign in and out takes place at the main entrance area of the classroom on a form. All areas identified on the facility sketch were toured and inspected both indoors and outdoors. Census was taken. LPA entered Classroom 1, which is an open concept classroom. LPA Perry observed 0 children and 4 staff present. LPA observed age-appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age-appropriate toys accessible to children. LPA observed a cubby for children to store their personal belongings. LPA Perry observed a step trash bin with a lid. All classroom doors were observed to have bells. Heating, lighting, and ventilation were evaluated by LPA Perry. LPA observed central AC/heater. LPA observed the thermostat located in classroom and temperature to be at 70 degrees. LPA observed the fire extinguishers located in classroom and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 8/27/2024. Smoke detectors were not tested during this visit due to the detectors being wired to the emergency pull down fire alarm system. Carbon monoxide detector in classroom and LPA Perry tested and are operable. The last emergency drill was conducted on 8/22/24. LPA observed napping equipment in the form of mats to be in good condition and stored in the classroom. First aid kit supplies are kept inside the classrooms and LPA observed the kits to be fully equipped. LPA reviewed medication. At this time, there is an area inside the classroom (library area) that is designated as the isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted and children will utilize staff restroom. Cameras were observed in the classrooms. LPA entered the restroom, LPA Perry observed the restrooms to be in good condition with working toilets, sinks, hand soap dispenser and paper towels. LPA did observe a wall diaper changing table. This program provides breakfast and lunch. Meals are provided by Plaza de la Raza main kitchen, who deliver the meals daily. LPA Perry informed mina that any food brought from the children's homes, the container shall be labeled with the child's name and properly stored or refrigerated. For water drinking, LPA Perry observed water jug and disposable cups in the classroom. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including movable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects, and rodents. LPA observed cleaning solutions stored in a cabinet with a lock making it inaccessible for children to reach and open in the off-limit kitchen area. The following documents were posted in a prominent, publicly accessible area at the facility: Facility License, Notification of Parents' Rights (PUB 393), Personal Rights (LIC 613A), Menu, and Earthquake Preparedness (LIC 9148). PUB 269 Child Passenger System Poster, and Daily Schedule. LPA Perry observed sign in/out. Transportation services are not provided at this facility. Children’s Records were reviewed to ensure that the following are present: LIC 613A Personal Rights, Admission Agreement, LIC 700 Identification and Emergency form, LIC 701 Physician’s Report, LIC 995 Notification of Parent’s Rights, LIC 627 Consent for Emergency Medical Treatment and Immunization Record. LPA Perry reviewed the children’s roster, children’s files and observed the files to be complete. L
