Based on observation, interview and record review, the licensee did not comply with the section cited above. During file review, LPA discovered that immunization records were missing for children 1 through 3. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee will submit copies of immunization records for children 1 through 3 to CCL by the POC due date.
Based on observation, interview and record review, the licensee did not comply with the section cited above. The Mandated Reporter Training certificate for the Licensee expired on 12/19/2025. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a current copy of the Mandated Reporter Training certificate to CCL by the POC due date.

Resolution: Licensee will submit a current copy of the Pediatric First Aid & CPR certificate to CCL by the POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 children files did not have LIC 995A Family Child Care Home Notification of Parent's Rights, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have parents complete and submit LIC 995A and licensee will send LPA proof by POC date 2/10/2025
Based on observation and record review, the licensee did not comply with the section cited above in 1 out of 3 total counts staff member did not complete mandated reporter training , which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee assistant will complete the training and will email the certificate to the LPA by POC date .
Based on interview and record review , the licensee did not comply with the section cited above and did not have First Aid training completed , which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee , she will enroll into the training and will email proof of enrollement by POC date and then will email completed certificate to the LPA .
Based on observations, interview and record review , the licensee did not comply with the section cited above and was unable to provide the proof of immunizations for the 2 assistants , which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email proof of immunizations for assistant 1 and assistant 2 by POC date to LPA.
