Resolution: Licensee will obtain fingerprint clearence for the two adults, and send LPA the Liv Scan by POC date.
Based on record review, the licensee did not comply with the section cited above in 1 out of 3 staff record reviews did not have current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will provided proof of current Mandated reporter training, by mail by POC date.
Based on observation, interview, record review, the licensee did not comply with the section cited above in not associating fingerprint clearence Yaffa Marcus to the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Yaffa Marcus filled out LIC 9182 Criminal Background Clearence Transfer Request and gave to LPA Morales at the time of the visit.
