Based on LPAs interview disclosure from Principal and records review of child #1 is missing written approval and instructions from the child's authorized representative for the administration of the Albuterol Sulfate medication, this poses a potential health and safety risk to children in care.

Based on interviews adn staffs' statements, Licensee did not sure that Child #1 was accorded dignity, as staff disclosed that Staff #2 pulled on Child #1 against their own volition. This is a potential risk to the health, safety, and personal rights of children in care.

(3) Prescription medications may be administered if all of the following conditions are met: (B) For each prescription medication, the licensee shall obtain, in writing, approval and instructions from the child's authorized representative for the administration of the medication to the child.
