Based on LPA obsesrvation and record review at 12:10 PM, LPA conducted record review and did not observe proof of completion of mandated reporter training for Licensee or two assistants present during today's inspection. Per Licensee, she states that she has not completed this training.This poses a potential heath, safety, and personal rights risk to children in care.
Resolution: Licensee states she will provide proof of completion of mandated reporter training for her and staff by POC date via email.
Based on LPA record review and interview, at 12:09 PM, LPA conducted record review and there were no records for staff available to review. Per Licensee she does not have staff files at this time, this is a potential health, safety, and personal rights risk to children in care.
Resolution: Licensee states she will gather staff documents and have it available for review by POC date via email.
Based on LPA record review and interview, at 12:09 PM, LPA did not observe proof of required immunizations for TdaP (pertussis) and MMR (measles) or Influenza (may provide written declination for this only) Licensee and two assistants.
Resolution: Licensee states she will provide photo proof of immunization records to LPA by POC date via email.
Based on record review and interview, at 12:18 PM, LPA conducted record review and C5 and C6 did not have any records available for review at time of inspection. Licensee states that both children just started and paperwork is currently with the parents to fill out. This poses a potential health, safety, or personal rights risk to children in care.
Resolution: Licensee states she will obtain records for children and provide photo proof to LPA via email by POC date.
Based on record review and interview, at 12:18 PM, LPA did not observe an LIC9227 available for C6 (D.O.B. 07/30/2021), which Licensee states is 2 months old.
Resolution: Licensee states she will have parent fill out LIC 9227 to keep in children's record and provide photo proof to LPA via email by POC date.
