Based on observation, the licensee did not comply with the section cited above, fire extinguisher are expired and not have been derviced since 2022 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will service it and submit a photo to LPA of service tag or receipt by POC dued date of 1/19/2024.
Based on observation, the licensee did not comply with the section cited above. Licensee had Mosquito Repellant and Lysol accessiable to the children which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee placed Lysol in a locked cabinet away and made inaccessible to the children and got rid of the Mosquito spray.

Resolution: Per Licensee, licensee and her assitants will complete the mandated reporter training by the POC due date 1/22/2023 and will submit completion certificate's to LPA.
