CHRISTOPHER AND EVANS FAMILY CHILD CARE

Based on observation by LPA Calderon: LPA observed at 1:59pm cleaning components , disinfectants and laundry detergents accessible to children: Kirkland detergent pods , 2 disinfecting bottles, Oxy clean and Shout cleaning components on top of the laundry machine in the kitchen accessible to the children in care, picture taken and additionally there was a Oxy Clean yellow bottle on the changing table in the living room the licensee did not comply with the section cited above in [6] out of [6] cleaning components, disinfecting products and detergants, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee during visit immediately placed cleaning components, disinfecting products and detergants in locked cabinets in kitchen and in locked bathroom cabinet. POC cleared during visit.

Based on LPA calderon's observation Fire extinguisher was observed, picture taken at 2:04pm, Licensee unable to provide receipt or service tag within the year and open wall face heater , in living room and in hallway observed screened only the bottom section, exposing the mid and upper section, picture taken at 2:02pm, the licensee did not comply with the section cited above in [2] out of [2] items (wall heater / fire extinguisher )which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will screen off the whole wall heaters in hall way and living room and licensee showed Amazon purchase of fire ext. with purchase date of 7/10/25. Licensee will send pictures of fire ext. and screen wall heaters by poc due date.

Based on observations LPA Calderon reviewed Licensee and Staff Assistant #1 Mandated Reporter certfication expired and per Licensee Angel Christopher Staff #2 has not done Mandated Reporter Training , the licensee did not comply with the section cited above in [4] out of [4] staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees (2) and (2) staff assistants will conduct Mandated Reporter Training- Child Care Providers and sent certfications to LPA Calderon by poc due date.

Based on observation and record review by LPA Caldero Children Roster was not updated LIC9040 was not complete missing children's address , children's physician information, children's authorized representative information, enrollment date and date left, picture taken of roster, the licensee did not comply with the section cited above in (1) out of [1] roster which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIC 9040 Roster will be completed and sent picture to LPA Calderon by poc due date.

Based on observation LPA Calderon during ecord review for staff #1 and staff 2 (staff assistants) did not have MMR and TDAP (DTAP) record in file, the licensee did not comply with the section cited above in (2) out of [2] staff immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: staff #1 and staff 2 (staff assistants) will obtain immunization records or get vaccination if needed and Licensee will send records to LPA Calderon of MMR/ TDAP (DTAP) by poc due date
