Resolution: Will not allow anyone in the home during operating hours that has not been fingerprint cleared. Licensee completed and submitted a compliance letter.
Based on observation the licensee did not comply with the section cited above in 1 out of 1 Fire Extinguisher did not have current service tag which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send photos of updated fire extinguisher.
Based on observation, interview and record review, the licensee did not comply with the section cited above in one out of one persons which poses an immediate health, safety or personal rights risk to persons in care the Licensee did not ensure that Jania Deleon had obtained a criminal record clearance prior to being able to work with children alone which posed an immediate safety and personal rights risk to persons in care.
Resolution: Licensee stated that the person mentioned above will not be working at the facilty any longer. Noone will be allowed to work without proper fingerprint clearance.
Based on record review, the licensee did not comply with the section cited above in 2 of 2 staff had expired Mandated Reporter Training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she and her assistant will complete the Mandated Reporter Training certificates and send copies to LPA by 01/31/2025 and put reminder in her phone.
Based on observation, the licensee did not comply with the section cited above due to a raised wood box planter located in backyard where the children play is tilting to one side and can possibly fall, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per license, the plantar will be removed or repaired, proof of correction will be emailed to LPA by POC due date
