Based on LPA’s observations and interviews conducted Staff did not provide a safe environment resulting in C2 who had previous similar incidents causing C1 injuries. This poses a potential risk to the health and safety of children in care.

Based on Lpa’s observations and interviews conducted C1 was disenrolled without 15 days prior written noticed given to parents as stated in enrollment agreement. This poses a potential health and safety risk to children in care.
