Based on LPA's record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that all of the childrens file contain current and updated immunization records prior to their attendance at the child care home.
Based on LPA Clayton's observation and record review, the licensee did not comply with the section cited above regarding conducting fire drills and disaster drill at least once a month, in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee is the ensure that disaster drills are conducted and documented according the Title 22 Regulations.
Based on LPA Clayton interview with staff, that the licensee did not comply with the section cited above regarding absences not exceeding 20 percent of the hours that the facility is providing care per day, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: As stated in the above mentioned Title 22 Regulation, Licensee is to ensure that she shall be present in the home and shall ensure that children in care are supervised at all times, and temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day.
Based on LPA Clayton's observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit the required current Mandated Reporter training certificates for her staff to the department no later than Thursday August 28, 2025.
Based on LPA Clayton's observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit the required current and complete CPR/First Aid training certificates for Child Care Providers for her staff to the department no later than Thursday August 28, 2025.
Based on LPA Clayton observation, the licensee did not comply with the section cited above as evidenced by the cabinet in the kitchen where which household cleaning products are stored has a broken child safety lock which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will install a new lock on the cabinet door, making the cleaning products inaccessible to children in care.
Based on LPA Clayton observation and record review, the licensee did not comply with the section cited above only 3 of the 6 children in care have a file on hand for review at the time of the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that all children in care have a complete file on site, at the FCCH which will be available for review by the department as requested.
Based on LPA Clayton observation the licensee did not comply with the section cited above as evidenced by mouse dropping in the bathroom cabinet, and the kitchen and refridgerator needing to be cleaned which pose a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure the the home is neat and clean and free of debris and vermin and/or rodents. Licensee will provide LPA Clayton with an inspection report from a pest control inspection no later than Monday July 8, 2024.
Based on LPA Clayton record review the licensee did not comply with the section cited above as evidenced by staff admission the they do do not conduct disaster drills with the children, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will immediately bgen to conduct fire, earthquake and disaster drill with the chidren and staff and keep a log of the drills, and make the log available for review by the department.
