Based on record review, the licensee did not comply with the section cited above in LPA was not able to verify Licensee's proof of immunization of measles which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, actions have been taken to verify of Licensee has immunization of measles. lab results are pending. Licensee will submit a copy of immunization of measles verified by physicians office and submit via email to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in LPA was not able to verify if Pediatric First Aid/CPR training Licensee and Assistant completed are valid. American Red Cross showed no record of course completed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, Instructor who facilitated the Pediatric First Aid/CPR training was contacted and advised to provide valid certificates. Licensee will submit copies of current and valid certificates for Licensee and Assistant #1 via email to LPA by POC due date.
