ANGELICOLA FAMILY CHILD CARE

Based on observation and interview, the licensee did not comply with the section cited above in kitchen counter top to be missing a piece of quartz which is exposing the wood and has sharp and pointed edges which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will immidiatley block off access to the kitchen via a kiddy gate until kitchen counter top is replaced. Licesee stated that she will have kitchen counter top replaced by POC date. LPA observed licensee put kiddy agte to block off kitchen area until counter top is fixed.

Based on observation, interview and record review, the licensee did not comply with the section cited above in staff not having mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee stated that she will have staff complete mandated reporter training and submit certification to LPA by POC date.

Based on observation, the licensee did not comply with the section cited above in 1 out of 2 staff members in that Staff #1's criminal background was not cleared and Staff #1 was not associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: After LPA deteremined that Staff #1 was not cleared and associated, Licensee had Staff #1 immediately vacate the facility premises. Licensee registered for Guardian and submitted necessary paperwork to have Staff #1 cleared and associated to the facility. Licensee will have parents of each enrolled child (and those who will enroll within the next 12 months of this report date) fill out and sign a LIC 9224 and email each copy to LPA by POC Due Date.
