ANAYA FAMILY CHILD CARE

Based on record review and interview, the licensee did not comply with the section cited above in licensee's assistant stated that October 24, 2024 was the last day the sleeping log was completed and Child #2 and Child #4 have been present at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant stated that they will start completing the sleep log by POC date.

Based on observation and record review, the licensee did not comply with the section cited above in the uncleared adult Sarah Maldonado was osberved by LPAs cleaning and providing care and supervision to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant asked uncleared adult to leave the facility. LPAs informed assistant that any adult that is present during day care hours must be fingerpint cleared and associated.

Based on observation, the licensee did not comply with the section cited above in LPAs observed knives to be placed in a drawer in the kitchen were the safety latch was broken, leaving the knives accessible to children in care. LPAs observed cleaning solutions under the kitchen where the safety latch was broken and the solutions were made accessible which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant stated she will inform the licensee of the repairs and have them fixed by POC date. Licensee's assistant moved the knives to a higher cabinet in the kitchen.
