Based on observation, the licensee did not comply with the section cited above in that LPA's observed a box of expired milk and a box of expired yougurt which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation and record review, the licensee did not comply with the section cited above in that facility did not inform the department of the Principal change within 10 days, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on information obtained during this investigation, the licensee did not comply in the section above which facility failed to provide a safe environment which caused C#1 who wandered off the facility premises. This poses an immediate health and safety risk to children in care.
