Based on observation, the licensee did not comply with the section cited above in Licensee stated that she purchased the fire extinguiser less than a year ago; however, Licensee couldn't find the receipt. Licensee attached the receipt from last years purchase 02/11/23, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, she will purchase a new fire extinguisher and will submit proof to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in Licensee and assistant have an expired Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, the Mandated Reporter Training will be taken and will submit proof to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in Child 6 (C6) did not have a completed LIC 9227 on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, she will provide LIC 9227 to parents of C6 and will submit proof of completion to LPA by POC due date. LPA provided consultation regarding LIC 9227.
Based on LPA's observations and interviews, the licensee did not comply with the section cited above in that Per Licensee the Fire Extinguisher has not been serviced in two years which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee she will buy a new fire extinguisher and submit proff to LPA by POC Due date.
Based on LPA's record review and interview, the licensee did not comply with the section cited above in that Licensee and Assistant do not have proof of the Mandated Reporter Certificate AB1207 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee- both licensee and assistant will take the training and submit proof of certificate by POC Due Date of 3/21/2022 via email.
