4TH STREET EARLY EDUCATION CENTER

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.

Responsibility for Providing Care and Supervision:The licensee shall provide care and sup.. as necessary to meet the children's needs.No child(ren) shall be left without the supervision of a teacher at any time,Supervision shall include visual obs.. This req is not met as evidence by Based on interviews the licensee did not comply in the section above in that 4 of 4 staff did not witness incident between child # 1 & child # 2 causing child # 1 hitting their head and lip req. medical attention.This poses an immediate risk for the health and safety of children in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that Staff # 2 does not have current mandated reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

.3(b)(1)California Lead Action Level at Child Care Centers. (b) Testing results with...readings of 0.5 ppb or greater.., before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not me at evidenced by Based on ispection, the licensee did not comply with the directive above, as that Sink faucet F, G, H, J and M tested with an Action Level Exceedance (ALE) of 6.01, 9.390, 8.360, 5.530 and 25.200 ppb. This poses a potential health and safety risk to children in care

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.

(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).
