Based on record review, the licensee did not comply with the section cited above as several personnel files were missing requried documentation which poses/posed a potential health, safety or personal rights risk to persons in care. Documents needed include current Mandated Reporter certificates, Proof of immunization against Measles (MMR) & Pertussis (Tdap), Proof of TB clearance, and Heath Screening Report (LIC 503)
