Based on record review, the licensee did not comply with the section cited above as 8 staff members either had expired or no current certification for Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as three staff members do not have active mandated reporter certifications which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as 5 staff members do not have a Health Screening Report (LIC 503) nor a TB Clearance, which poses/posed a potential health, safety or personal rights risk to persons in care.

One outdoor drinking fountain attached to the handwashing station, whichthe facility never used for drinking or food preparation, was tested for lead and the results showed a level of 8.510 ppb, which poses a potential health, safety, or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as 6 staff members do not have proof of immunization for Measles (MMR), Pertussis (Tdap), or both, which poses/posed a potential health, safety or personal rights risk to persons in care.
