ZARATE FAMILY CHILD CARE

Based on interview, record review, and observation, Licensee and Staff EMSA Pediatric CPR and First Aid expired on 01/20/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Staff will obtain appointments together and send the copy of the certificates to LPA's email by 05/15/2025.

Based on interview and record review, Licensee did not notify LPA when a new adult moved in the home, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA trained staff about reporting requirements and provided the MPSW incident reports and the LIC624B. The licensee and staff understand the reporting requirements. The adults were fingerprint cleared but did not have tB clearances on file

Based on interveiw, observation and record review, the fire extinguisher was not serviced per licensee, licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will replace or service the fire extinguisher and provide proof to LPA via email. Photo of tag or receipt.

Based on interview, observation, and record review, the staff, licensee, and adults in home need immunization records, the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LPA with missing immunization records for licensee, staff , and adults in home by June 2. Licensee will email the documents to LPA by June 2.

Based on observation and record review of children's files, there was no proof of enrollment in the files, , the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Contact parents and obtain proof of enrollment and place in the files, notify LPa when records are complete.

Based on observation, there is no working carbon monoxide detector, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a new carbon monoxide detector and install by May 13, 2023 and send proof to LPA.

Based on observation and record review, chlid #1 did not have a sleep chart implemented, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will implement the sleep chart 05/09/23 for child #1 and in two weeks, show proof to LPA

Based on observation and record review, Staff #1 and Licensee do not have mandated reporter training certificates, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Staff and Licensee will complete the mandated reporter training and email proof to LPA when completed

Based on observation and record review, staff and licensee do not have records of immunizations available during inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Staff will provide LPA with proof of immunizations and create a file with the records.

Based on observation and record review of three children's files, 2 out of 3 files did not have immunization records, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will check all of the records in files and use the Licensing form 311D, make sure all files are completed and show proof to LPA.
