This is a single license preschool program. LPA was met by Sarah Soriano, Executive Director. Site Supervisor Chriseta Champion guided analyst on a tour of the facility. During the inspection, LPA observed 6 staff with 33 children upon arrival. Classroom #1 with 2 staff and 15 children, Classroom #2 with 2 staff and 7 children, and classroom #3 with 2 staff and 11 children. The center was observed to be clean and free of defects. During the inspection, LPA conducted interviews, took photos and videos, obtained video of the incident on 08/11/25, reviewed records, obtained a weekly attendance sheet and LIC9040 children’s roster. Site supervisor Chriseta Champion self-reported the incident to CCLD on 08/11/25 as required by reporting requirements. Interviews conducted indicate that the preschool self-reported the incident that a child wandered away from the childcare center due to lack of supervision on 08/11/25 at approximately 10:10am. LPA observed the surveillance video footage of the incident. Per Executive Director, Child #1 will still be enrolled at the center and the parents are satisfied with the outcome. Based on LPA's observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 and Chapter 1, Article 6, Section 101229(a)(1) are being cited on the attached LIC 9099D. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA Dayna Chambers informed Executive Director Sara Soriano that this report dated 08/13/25 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.
