Based on observation, record review, and interview, Staff #2 did not have a criminal record clearance or association with the facility, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Staff #2 was sent home and licensee will obtain new prints for the staff before they start working again. Licensee will watch background check video and answer questions. Licensee will provide clearances to LPA via email by July 7th, 2025. LPA advised licensee that Staff #2 can't work until completed and proof is provided. Videos: ccld.childcarevideos.org

Based on observation, record review, and interview, fire extinguisher tag expired on 04/26/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will service the fire extinguisher after reopening from holiday. The licensee will email LPA by 07/11/25 proof of service tag.

Based on interview and observation, no staff files were available during inspection including immunization records, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will be closed for holiday, Licensee will complete all files including immunization records by 07/11/2025. Licensee will notify LPA when the files are completed.

Based on observation, record review, and interview, licensee's files were not available during inspection including fingerprint clearance documents, the licensee did not comply with the section cited abovewhich poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete all staff files and documents including criminal record clearance documents by 07/11/2025. Licensee will provide proof of correction to LPA by 07/11/25.

Based on observation, record review, and interview, children's files were not available during inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete all children's files including immunizations by 07/11/2025. Licensee will provide proof to LPA. Manner of proof will be determined either in person or phone call.

Based on observation, record review, and interview, the roster was not completed during inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
