Based on observation and record review, the roster was missing one enrolled childs information, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Add missing child's information to the roster and email updated roster by 08/04/23 to LPA

Based on observation and record review, children's files LIC700 was not complete and one file was missing, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Using the 311D, go over each childrens file and make sure all fields are entered, signed, and dated. Create the missing child's file. Email proof of correction to LPA by 08/28/23.

Based on observation of the backyard, LPA observed 2 gas BBQ's, 2 Propane Tanks , Charcoal Bag, Slide needs padding at end of slide due to cement, The gate lock is broken and fell of the hinge - needs repairs and make sure it locks, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove the items listed above by August 8, 2023. LIcensee will email LPA photos of the corrections. Licensee will keep the backyard off-limits until the items are corrected.

Based on observation and record review, the assistant is not documenting 15-minute safe sleep documentation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant was provided with safe sleep resources. Assistant will re-train all staff and watch provider videos. Website: ccld.childcarevideos.org All staff and licensee will document the date and signature, list the titles of videos watched and email the document to LPA by 08/28/2023.

Based on observation and record review, 1 childs did not have a file completed, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Using the 311D form, create the missing child's file by 08/04/23 send proof to LPA

Based on observation and record review, the sleep charts were not implemented for two infants enrolled, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Implement Sleep Charts and email proof to LPA by 08/14/23 for infants enrolled.
