Based on observation and record review, the fire extinguisher was not serviced and expired on 04/01/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have the fire extinguisher serviced or buy a new fire extinguisher. The licensee will email LPA a copy of the new receipt of photo of the new service tag by 04/25/25.
Based on observation and record review, the last emergency drill was conducted on 08/14/2024, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will conduct an emergency drill every six months and document. The licensee will conduct a new drill and send proof of documentaiton to LPA;s email by 04/25/25.
