Based on observation, record review, and interview, infant #1 did not have documentation of any sleep charts in the file, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will implement a sleep chart and place the completed sleep charts in the childs file.
Based on record review, interview, and observation, Infant #1 enrolled at 11 months of age. Licensee did not implement a sleep chart for infant #1, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee will implement infant #1's sleep chart and email proof of correction by 10/27/25.
Based on observation, record review, Assistant Ana does not have a CACI clearance, fingerprints are pending in LIS and Guardian, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA obtained copies of pertinent fingerprinting documents, licensee will contact CPMB registry research. LPA provided a copy of TITLE 22 fingerprint clearances to licensee.
