Based on observation and document review, the roster was not updated with current children, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will update the roster and email to LPA by 04/25/2025.
Based on observation whlie infant was sleeping, there were toys and blankets in the cribs, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee corrected the deficiency while LPA was in napping room. LPA advised licensee and assistant of safe sleep regulations.and explained SIDS.
Based on observation and file review, the licensee did not comply with the section cited above and mandated reporter for three individuals were expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will update mandated reporter certificates and provide LPA with the certificates by 08/01/22.
