Based on observation and record review, the fire extinguishers were not serviced within a year. The date of expiration is 09/03/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have the fire extinguishers serviced and the licensee will email a photo of the new service tag by 02/23/2026.

Resolution: The licensee and staff #1 will renew their mandated reporter training and show proof of correction via email by 02/23/2026.
Based on observation record review, and interview with licensee, Infant #1 did not have a sleep chart available during the inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will implement the sleep chart and email a copy to the LPA's email address by 02/23/2026.
