Based on record review, observation,,LPA observed an empty file for Infant #1. There were no documents in the file. the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the infant file with all missing records using the LIC311D form provided.

Based on record review, observation, and interview, 7 children that were present at the daycare today 10/07/25, were not on the roster, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: the licensee will update the LIC9040 children's roster and notify LPA when completed.

Based on record review and observation, Infant #1 was enrolled on 07/15/2025. The age of the infant upon enrollment was approxmately 9 months plus a couple weeks, LPA did not observe a sleep chart or LIC9227 in the infants empty file, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will create a sleep chart and log napping. LPA will clear deficiency in an in person visit.

Based on observation, inspection, record review, LPA observed the licensee with 11 children and no assistant on 10/07/25 at 1:15pm, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The children are leaving and licensee will not operate without an assistant when over 8 children are present.

Based on observation, record review, and interview, the fire extinguisher stated last service was 09/05/2024 and on 09/05/2024 expired not serviced, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will service the fire extinguisher and email LPA the new service tag by 10/14/25.

Based on record review, observation, and interview, the licensee's mandated reporter traiining expired on 08/06/25, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: the licensee will renew the mandated reporter training and email the renewed training to LPA by 10/17/2025.

Based on observation, record review, and file was empty, 7 out of 7 children's files was missing immunization records, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will complete the empty files and obtain immunization records for all children's files missing immunization records. The licensee will notify LPA when records are completed. LPA will come out for a case management to review records.

Based on observation, record review, and interview, 7 children's files were empty and missing the Emergency Form LIC700, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will go through all of the children's files and complete the missing licensing forms - LIC700 - and notify LPA when completed.

Based on observatoin and record review via Guardian, the licensee did not comply with the section cited above in count 2 LPA observed the volunteer was not cleared and working on 08/26/22 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Volunteer Brittney was removed from home and will not return until receiving clearances. The volunteer printed on 08/29/22.

Based on interview with the licensee did not comply with the section cited above in the licensee occasionally is not sleeping at the licensed home consistently which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a Plan of Correction and declaration - options - change of location or alternative plans to continue the current location. Licensee will provide the declaration and POC by September 5, 2022.
