Based on observation, the licensee did not comply with the section cited above LPA observed area in play yard that needs to be barricaded and made inaccessible to children - area is obscured from supervision, green storage door is broken and unable to lock which poses/posed a potential health, safety or personal rights risk to persons in care.
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.

(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidence by: Based on record review the licensee did not ensure water source at facility meet lead requirements. Lead test results showed that one water source has an action level exceedance of 7.2. LPA observed water sources are inaccessible to children.
