Based on record review, the licensee assistant did not comply with the section cited above, all staff did not have required Mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care
Resolution: Licensee will have assistant complete Mandated reporter training and submit certificate as proof of completion to LPA via email by agreed upon date.

Resolution: Corrected during inspection. Licensee opened the door where child was sleeping. The bedroom door has a half door. Per Licensee the door will remain open when infants are sleeping in the bedroom.
Based on observation, the licensee did not comply with the section cited above. The fire extinguisher did not met the 2A10BC standards required by the State Fire Marshall which poses a safety risk to persons in care.
Resolution: Per Licensee, will purchase 2A10BC by POC due date of 08/24/2022 and submit proof to LPA via email.
Based on observation, the licensee did not comply with the section cited above. There was 5 infants present during today's inspection which poses an immediate health and safety risk to persons in care.
Resolution: Per Licensee, will develop a scattered schedule for (2) infants and will talk to parents tonight 08/10/2022. Per Licensee if she cannot come up with a scattered schedule that works, she will disenroll (2) infants. Licensee will provide a plan to LPA via email and will follow up with LPA via cell phone on 08/11/22.
Based on observation and record review, the licensee did not comply with the section cited above. On 07/15/22, assistants were not able to locate any files which poses a potential safety risk to persons in care.
Resolution: The observation were corrected and not observed during today's inspection. During today's inspection, all children's files were current and up to date.
Based on observation, the licensee did not comply with the section cited above. LPA observed in bedroom#2 an infant sleeping in a FISHER PRICE Rock n Play sleeper, which have been recalled since 04/12/2019 by the United States Consumer Product Safety Commission, which poses an immediate personal rights risk to persons in care.
Resolution: Assistant removed infant from the sleeper. Per assistant they will inform Licensee that the sleepers are recalled and are not allowed. LPA reviewed the safe sleep regulations with assistant. Assistant will call Licensee on 07/18/22 to see if she can throw away sleepers. LPA will follow up on 07/19/22.
Based on observation, the licensee did not comply with the section cited above. Upon arrival, LPA observed 16 children present which poses an immediate safety risk to persons in care.
Resolution: Per assistant, she will offer parents alternate schedules. Assistant will talk to Licensee as soon as possible to come up with a plan. If issue is not solved by next business day, facility will closed until a solution is found. LPA will follow on 07/19/22 by phone.
Based on observation, the licensee did not comply with the section cited above. Two infants were asleep in bedroom #2. The door was closed and there where no adults in the room watching the children. LPA did not observe any monitors and there was no documentation of assistant checking on the infants every 15 minutes. Which poses an immediate health, safety and personal rights risk to persons in care.
Resolution: Per assistant, infants will continue to sleep in bedroom #2 with the bedroom open. Licensee will submit a sleeping plan for the infants. Plan is due 07/29/22.
