Based on interview, record review and inspection, the fire extinguisher has not been serviced and/or the licensee does not have proof of receipt during this inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will either service the fire extinguisher or obtain the receipt for the fire extinguisher from Costco, or if purchasing a new extinguisher, a receipt or service tag photo will be emailed to LPA for proof of correction by 08/25/2025.

Based on record review, observation, and interview, the assistant does not have proof of immunizations available during this inspection (MMR, TDAP, TB Clearance, influenza or declination) , the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The assistant will provide licensee with copies of the MMR, TDAP, current TB clearance, and influenza or declination by 09/01/25 and email the copies to the LPA by 09/01/25.

Resolution: Licensee will request copies of the immunization records from the assistant by September 1 and email the documents to LPA;s email provided.

Based on inspection and interview, there is no carbon monoxide detector in the home during this inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will purchase a carbon monoxide detector and provide LPA a copy of the receipt, a photo of the detector, and a video to show that it works and is operable by 08/25/25. Can text message or email the corrections.

The assiistant has been working at the home for a year, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
