Resolution: Licensee will replace the carbon monoxide detector and send me a video to my cell phone- provided licensee with my business card.
Based on observation and record review, the fire extinguisher receipt expired on 10/04/2024, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will text the new service tag by February 3, 2025.
Based on observation and record review, staff assistants (2) did not have the mandated reporter training, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Assistants will take the training and provide the certificate to LPA by 02/28/24 via email.
Based on observation and record review, there were no files available to review with immunization records for licensee and staff. The licensee and staff do have immunizations that will be requested from offices, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will create new files for staff that includes all missing records by 02/28/25. Licensee will contact LPA when completed to obtain proof.
Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee has not serviced the fire extinguisher since March 2022.
Resolution: Per Licensee, they will purchase a new 2A-10BC fire extinguisher and provide LPA a copy of the receipt to LPA by the POC date of 9/08/23.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee did not have a current Mandated Reporter training on file and stated that she has not renewed her training. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee will renew the AB 1207 Mandated Reporter training by POC due date 02/24/23 via email.
