Based on observation, record review, and interview, the last fire drill conducted was 04/04/2025. The drill should have been conducted again on 10/04/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, the fire drill will be conducted and updated on the fire drill log by 11/21/2025. The licensee will email proof of correction via email.

Based on observation and record review, the fire extinguisher service tag expired 08/19/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will renew the service tag / service the fire extinguisher by 11/21/25 and email proof of correction to LPA.

Based on interview, record review, and observation, the assistants mandated reporter was expired on 08/21/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per staff, the mandated reporter will be renewed by 11/21/25 and proof of correction will be emailed to LPA.

Based on observation, record review, and interview, licensee and staff's cpr expired on 08/03/2025, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Staff will renew their CPR and provide proof of correction to LPA via email by 11/21/2025.

Based on observation and review of fire extinguisher, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will service the fire extinguisher and send proof by 08/16/24 by email.

Based on record review, the licensee did not have a current mandated reporter certificate for department review during inspection on 11/09/22, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send the LPA the mandated reporter certificate after completing the class by 11/14/22 by email

Based on observation and testing, The smoke detectors were defective and the fire extinguisher was not serviced, the receipt was expired on 04/11/2022, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will replace the smoke detectors and fire extinguisher today, 11/09/22. Licensee will provide proof by photos and video of the fire extinguisher receipt and the smoke detector replacement- will send a video of testing to LPA
