Based on record review, the licensee did not comply with the section cited above in 1 out 2 staff records missing immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee will obtain a current record of their immunzation records. The Licenee will place a copy of the immunization record in the file. The Licensee shall provide proof of the immunization record to LPA on or before the POC due date.
Based on record review, the licensee did not comply with the section cited above in 5 out of 5 children files which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will let parents know as of 8/8/2024 will send text blasts to parent requesting for new immunizations. Licenseee will send proof immunizations via email to LPA Dunlap by the POC date.
