Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 count record review which poses/posed a potential health, safety or personal rights risk to persons in care. Last drill was conducted on 12/6/2025 and was not being conducted every 6 months.
Resolution: Licensee will conduct fire and disaster drill awith the children nd provide written documentation to LPA for observation. An email copy will be sent to LPA.
Based on record review and interview, the licensee did not comply with the section cited above in 1 out of 1[(objects) record review which poses/posed a potential health, safety or personal rights risk to persons in care. Assistant is missing MMR vaccination records.
Resolution: Licensee will discuss with the assistant in obtaining the MMR vaccination record. Licensee will send a copy of the MMR vaccination record to LPA once completed.

Resolution: Licensee will deliver the LIC 627 forms for all parents to sign. Licensee will then email a copy to the LPA for observation.
Based on observation and record review, the licensee did not comply with the section cited above in 1 out of 1 [(objects) person which poses an immediate health, safety or personal rights risk to persons in care. Assistant is not fingerprint cleared.
Resolution: Licensee will discuss with assistant that the assistant can not return to work or to the facility until the fingerprints and livescans are completed and cleared. Once assistant's fingerprints are cleared, licensee will send LPA the clearance letter for observation.
