Resolution: Licensee to completed the mandated reporter training certification and send via email to elizabeth.george@dss.ca.gov no later than July 25, 2025.
Based on observation, the licensee did not comply with the section cited above in that fire extinguisher expired on 3/4/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to service her fire extinguisher and submit proof via email at francisca.velazquez@dss.ca.gov by 4/19/23.
