Based on observation, interview, and record reviews, the licensee did not comply with the section cited above in 2 count out of 2 count objects, persons missing 15 minute infant sleep checks which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided a copies of 15 minute infant sleep check charts. Licensee will begin documenting starting immediately, 11/20/2025 and will provide LPA picture proof that 15 minutes infant sleep checks are being conducted daily for each infant in care.
