Based on observation the licensee did not comply with the section cited above in having one rectagler swing in the back yard which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The LPAs observed the licensee elevated the swingand attached to the play structure. Licensensee tangled the swing on the overhead wodden swing hanger in order to prevent access to children.
Based on record review the licensee did not comply with the section cited above in one out of 9 children in care.C8 was not listed in the roster.The roster also has incomplete section pertaining to the facility number, date and names. Which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee needs to input the required fields in the facility roster. Once the completed roster with the required information has been inputed the licensee will scan and email the document to Licensing Program Analyst: German.Negrete@dss.ca.gov
