Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 1 fire extinguisher which poses/posed a potential health, safety or personal rights risk to persons in care. Fire extinguisher was purchased over 1 year and needs to be replaced or serviced.

Resolution: Licensee will purchase or service a fire extinguisher and show proof of receipt to LPA.

Based on observation, the licensee did not comply with the section cited above in 1 out of 1 count which poses/posed a potential health, safety or personal rights risk to persons in care. Body lotions, room spray, perfumes, skin care items were located in a bathroom shelf accessible to children in care.

Resolution: Licensee will remove all skin care items and perfumes out of the bathroom to a higher top shelf or inside a cabinet with a child's lock. LIcensee will take photo proof and email to LPA.

Resolution: Assistant will go to the doctor to retrive updated immunization record that shows the MMR and TDap immunization was completed along with the TB test. A photo of the immunization record and results of TB test will be email to LPA for review.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 2 out of 2 people do not have a valid certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to submit valid certificate to LPA.

Based on observation, the licensee did not comply with the section cited above when two exersaucers were observed in the home which are prohibited equipoment in licensed day care homes. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall remove the items from the home by 12/17/2021.

Based on observation and record review, the licensee did not comply with the section cited above the 2A10 BC Fire Extinguisher was purchased on 10/30/2020. The fire extinguisher was not serviced or replaced yearly as required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall purchase or service 2 A10 BC Fire Extinguisher and submit verification to Licensing for review by 12/20/2021.
