Based on observation and interview, the licensee did not comply with the section cited above in [count] out of1 object found which poses/posed a potential health, safety or personal rights risk to persons in care.Gardening tools, such as shovels, gardening scissors, and sharp cutting objects were found in a box in the play yard. Cleaning agents were found as well inside the box.
Resolution: Licensee will remove the box of gardening tools and cleaning agents out of the outdoor play yard into a locked storage room. Licensee will take a picture and send the correction to LPA for observance.

Resolution: Licensee will provide parents of infants with individual infant sleep plan and provide a finished copy to LPA for record review.
Based onobservation , the licensee did not comply with the section cited above in one out of one observation which poses/posed a potential health, safety or personal rights risk to persons in care. Infant in care sleeping was in a closed room with a blanket in the crib. 15 minute checks were also not conducted.
Resolution: Licensee will remove the blanket from the infant sleeping in the crib. THe licensee will also conduct the 15 minute checks moving forward and leave door open while infant in care are sleeping. Licensee will take a picture of the 15 minute sleep check and door opened for LPA to observe.
