Based on observation, LPAs observed two trash cans without lids filled with garbage and a bag of Scotts Turf Builder Weed and Feed in the cubie within children's reach at the entrance of the daycare facility. LPAs also observed a large container with open top filled with rain water and chicken excremen accessibleto children; therefore, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to remove the trash cans, bag of Scotts Turf Builder Weed and Feed, and a large plastic container filled with rain water and chicken excrement from the areas which are accessinle to the children. Licensee also agrees to submit to LPA the signed LIC9224s upon completion.

Based on record review and intervuew with licensee, licensee's mandated reporter expired on 2/19/2025, and licensee did not have a valid mandated reporter training; therefore, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the child care provider mandated reporter training by the POC due date (2/9/2026). Licansee also agrees to submit a copy of the certificate of child care provider mandated reporter training to LPA upon completion.
