Based on observation, the licensee did not comply with the section cited above, there was loose sheet placed on the top of the play pen to provide shade, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall remove the loose sheet immediately and will review the Safe Sleep video on https://safetosleep.nichd.nih.gov/resources/videos titled Safe Infant Sleep. Licensee will submit a statement how she will maintain the safe sleep regulations.
Based on record review, the licensee did not comply with the section cited above in, licensee does not have 15 minute check log, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall document and provide a copy of 15 minute check sleep log to LPA by 6/25/2025.
Based on observation, the licensee did not comply with the section cited above. The kitchen drawer has medicine. Under the sink in the day care room has cleaning supplies, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee moved the medicine and locked the cabinet under sink with cleaning supplies.
Based on observation and record review of the fire drill log, the last fire/disaster drill was conducted more than 6 months ago which poses a potential risk to children in care.
Resolution: Licensee shall conduct a fire/disaster drill and submit copy of drill log to LPA by 10/12/22.
Based on observation, the licensee did not comply with the section cited above. This poses a potential health, safety or personal rights risk to persons in care. LPA observed an 2 infants sleeping in a cribs with loose items present in the crib (a blanket and a pillow).
Resolution: Licensee shall review Safe Sleep regulations and submit to LPA a written plan detailing how infants will be placed in cribs to sleep in compliance with Safe Sleep regulations. Licensee shall submit photograph example of how Safe Sleep regulations regarding cribs have been met and submit to LPA by 10/12/22.
